For UK patients, the authorised Mounjaro presentation covered by the current UK product information is the multi-dose KwikPen. Each pen is designed to deliver four fixed doses of the strength shown on its label. Do not assume that single-dose vials sold or described in other countries are an authorised UK presentation, and do not try to extract a fifth dose from the pen.
UK presentation
The UK product information describes the multi-dose KwikPen.
Four fixed doses
Each pen is designed for four 0.6 mL doses.
Use a new needle
Attach a compatible new pen needle for each injection.
No fifth dose
Discard the pen after its four intended doses; do not withdraw residual liquid.
What the UK KwikPen contains
The Mounjaro KwikPen is a pre-filled, multi-dose pen. Each pen contains four doses of the strength printed on the label. The available strengths include 2.5 mg, 5 mg, 7.5 mg, 10 mg, 12.5 mg and 15 mg per 0.6 mL dose. A pen is not a multi-strength device: it delivers the labelled strength and should not be manipulated to create another dose.
The UK patient leaflet explains that a small amount of solution may remain after the four doses have been correctly administered. This residual amount is expected and is not an additional dose. The instructions say not to try to use it; discard the pen after the final intended dose in accordance with local sharps and medicine-disposal advice.
Some international product information may describe other presentations. Availability and authorisation differ by country and can change. This article concerns the UK KwikPen information and should not be used to guide use of an overseas device.
Why comparisons with single-dose vials can mislead
A single-dose vial requires a different device and administration process from a pre-filled pen. The UK KwikPen instructions do not authorise drawing medicine out of the cartridge with a syringe, and they do not provide a dose-conversion table for vial presentations. Advice intended for a different country, product pack or device should not be transferred to the UK pen.
Online posts sometimes compare the KwikPen with vials and describe manual syringe measurement as a way to obtain extra doses or lower costs. That is not an approved way to use the KwikPen. It bypasses the manufacturer’s dosing mechanism and creates avoidable uncertainty about dose measurement, contamination and device handling.
If a patient receives a product that is not the expected UK KwikPen, they should not inject it until a pharmacist or prescriber has confirmed what it is, whether it is authorised for the UK market and how it should be used.
Needles, injection sites and safe handling
The KwikPen requires a compatible screw-on pen needle, which is not included in the pack according to the product information. Use a new needle for each injection and follow the pen’s instructions for attaching, priming, selecting and administering the dose. Do not share a pen or needle with another person, even if the needle is changed.
The medicine is injected under the skin of the abdomen, thigh or back of the upper arm. Another person should administer an injection into the back of the upper arm. Rotate injection sites and avoid injecting into skin that is tender, bruised, scarred or damaged. If insulin is also injected, use a different injection site.
After the injection, remove the needle and dispose of it safely in an appropriate sharps container. Do not store the pen with a needle attached: the product instructions warn that this can cause leakage, blockage or air entering the pen. Keep the pen capped and follow the leaflet for storage.
The fifth-dose extraction risk
The amount remaining after four correct doses is not intended for injection. Do not use an insulin syringe or other device to withdraw it. The pen is validated and labelled for four doses; attempting to obtain more medication is outside the instructions and can lead to uncertain measurement or contamination.
If the pen appears not to deliver a full dose, do not compensate by guessing or repeating the injection. Consult the patient leaflet and contact the dispensing pharmacist or prescriber for advice. If the pen is damaged or malfunctioning, retain it and its packaging while seeking advice.
