Long-Term Mounjaro Maintenance and Discontinuation: Weight Regain Evidence, Cost Planning, and Tapering

Tirzepatide can help with weight reduction while treatment continues, but the SURMOUNT-4 withdrawal trial found substantial weight regain among participants switched to placebo after an initial treatment period. The trial does not show that every person must take Mounjaro forever, does not establish a tapering schedule, and does not support stretching injections to every 10–14 days. Decisions to continue, pause or stop should be made with the prescriber.

SURMOUNT-4

Participants switched to placebo regained an average 14.0% of body weight from randomisation to week 88.

Trial context matters

Participants first received 36 weeks of tirzepatide and had no diabetes.

No proven taper schedule

The trial did not test an 8–12-week taper.

Do not space doses yourself

The authorised regimen is once weekly.

What SURMOUNT-4 actually studied

SURMOUNT-4 was a randomised withdrawal trial in adults with obesity or overweight and a weight-related complication, excluding people with diabetes. Participants first received open-label tirzepatide for 36 weeks, titrated to a maximum tolerated dose of 10 mg or 15 mg once weekly. The group entering the randomised phase had achieved a mean weight reduction of 20.9% during that initial period.

At week 36, participants were assigned either to continue tirzepatide or switch to placebo for another 52 weeks. From week 36 to week 88, the group switched to placebo regained an average 14.0% of body weight, while the group continuing tirzepatide lost a further 5.5%. These figures describe changes during the randomised phase, not a claim that every participant regained two-thirds of the weight they had lost.

The findings support the importance of planning for weight maintenance after initial weight loss. They do not establish the exact outcome for an individual patient, people with type 2 diabetes, people taking different doses, or people who stop for different reasons.

Does the evidence mean treatment must continue forever?

No single trial can establish that every person must remain on treatment indefinitely. SURMOUNT-4 shows that, in the studied population, stopping tirzepatide and switching to placebo was associated with substantial average weight regain over the following year compared with continuing treatment.

Obesity is often managed as a chronic condition, and weight can increase again after active treatment ends. However, duration of treatment should be reviewed individually, taking account of benefits, adverse effects, other health conditions, patient preferences, treatment goals and the available alternatives. The prescriber should explain the likely trade-offs rather than promise that weight loss will be permanent or state that lifelong treatment is mandatory for everyone.

Stopping or pausing treatment

The SURMOUNT-4 trial was not designed to test a gradual dose-reduction or tapering programme. It therefore cannot be used as evidence that reducing the dose over 8–12 weeks prevents weight regain or makes stopping safer. Do not follow a tapering schedule from an online article unless it has been recommended by the treating clinician for the individual situation.

If tirzepatide is used for type 2 diabetes, stopping may affect blood glucose control and an alternative diabetes plan may be needed. People should discuss the timing of discontinuation, glucose monitoring and any replacement treatment with their diabetes team. If stopping because of side effects, pregnancy planning, surgery, cost or supply problems, explain the reason to the prescriber so that the plan can address it.

Why extending the interval is not an established maintenance strategy

The authorised dosing schedule is once weekly. The product information does not establish that injecting every 10 or 14 days is an effective or safe long-term maintenance regimen. Nor does SURMOUNT-4 test extended intervals. It is therefore not appropriate to claim that such spacing has a proven effect on drug concentrations, appetite or gastrointestinal adverse effects in every patient.

Do not split doses, use leftover KwikPen liquid or alter the injection interval to reduce costs. If the prescribed treatment is difficult to afford or obtain, contact the prescriber or pharmacist to discuss appropriate options.

Planning for weight maintenance

Weight management should not rely on medication alone. NICE guidance describes tirzepatide as part of a broader approach that includes a reduced-calorie diet, increased physical activity and appropriate behavioural support. The practical plan should be realistic, tailored to the person and reviewed over time.

Useful discussions with the clinical team include what outcome is being monitored, how side effects are managed, how other conditions and medicines affect the plan, what happens if treatment is stopped, and what support is available for maintaining changes. A maintenance plan should be agreed rather than inferred from a trial average.

Frequently asked questions

How much weight did participants regain in SURMOUNT-4?

Participants switched to placebo regained an average 14.0% of body weight between week 36 and week 88. This is not the same as saying every participant regained two-thirds of their initial weight loss.

Does everyone need Mounjaro for life?

The trial does not prove that. It shows substantial average regain after withdrawal in the population studied. Treatment duration requires individual review.

Is there an evidence-based 8–12-week taper?

SURMOUNT-4 did not test a tapering schedule. Do not treat a specific taper as established evidence.

Can I space injections every 10–14 days?

That is not the authorised weekly schedule and is not an established maintenance regimen. Discuss affordability or interruptions with the prescriber.
